CONCLAVE REGISTRATION

Deadline June 15 – $10.00 Late Fee after June 15. Please register with your chapter **Pay late fee for all those after June 15

• LOCATION:  Al Menah Shrine Center 1354 Brick Church Pike•Nashville, TN 37207
• DIRECTIONS:  I-65 N West Trinity Lane Exit 87 (W. Trinity La.) to Brick Church Pike
• LODGING:  LaQuinta Inn 2345 Atrium Way Nashville, TN 37214. We are requesting that each individual call Group Reservations Department at 1-866-527-1498 and identify themselves as members of the TN DeMolay Conclave 2017, Reservation Block confirmation number GRYGYB for LA QUINTA INN NASHVILLE, BRILEY PARKWAY. Please also state the room type that you are requesting, arrival and departure dates as well as smoking preference by 6/15/2017. After this date the rooms will be released to make room for other guests and future reservations will be subject to rate and availability. *FREE BREAKFAST at the LaQuinta Inn

CONCLAVE PACKAGE FEES:

• $90.00 Thursday Arrival Includes:  Thursday-Dinner, Friday-Lunch & Dinner, Saturday-Lunch & Banquet
• $70.00 Friday Arrival Includes:  Friday-Lunch & Dinner, Saturday-Lunch & Banquet, $15.00 Honors Night Banquet only for Honorees & their guests
• $50.00 Saturday Arrival Includes: Saturday-Lunch, Banquet

If you are attending activities you will be expected to pay the respective fee for your arrival date or Registration fee of $20 plus meals. PACKAGE FEES ARE A BETTER VALUE!!

All those attending conclave must use lodging at LaQuinta or have approval of the Executive Officer. All those attending are expected to pay the conclave fee. Extra banquet tickets will be available as noted on the registration form. DEADLINE FOR THIS INFORMATION is June 15, 2017. $10.00 per person late fee after June 15. Please no phone in registration. Reservations only guaranteed if registered by the deadline.

• SWEETHEART COMPETITION:  Contact Mom Whitaker at 615-847-5075 and she will send you complete information. Sweetheart candidates must be between the ages of 14-21. MUST CONTACT MOM WHITAKER BY June 15 IF YOU WILL COMPETE.
• RITUAL COMPETITION:  See Attached. Questions, contact Dad Nathan Rogers at nathanrlrogers@yahoo.com
• SPORTS:  As always we will have an outstanding line up of sport activities. Held on Thursday this year at Al Menah Shrine
• PARENTAL CONSENT: All participants under the age of 21 are required to submit a parental consent /medical release form, Please use the master enclosed and make copies as needed
• EXTRA BANQUET TICKETS: A very limited number available on Thursday of Conclave. $30.00 per ticket Pre registered. $35.00 Late if available. We will assign a table for all attending the banquet. It is important to order all on the form to have seating together.
• CANDIDATES FOR STATE OFFICER: Please submit a letter of intent to Dad Whitaker by July 1 Separate e-mail will be sent on this topic
• MISCELLANEOUS:  All chapters MUST Have an advisor in attendance & Insurance Paid. Remember to to bring a few dollars for the DeMolay store & to buy a ticket for a dance with your favorite sweetheart.
